Hematological and Clinical parameters in patients of Chronic Kidney Disease (Hemodialysis dependent vs Drug dependent)

Abstract

Objective: To assess the hematological & chemical parameters in association with clinical findings in patients of chronic kidney disorder (hemodialysis dependent compared with drug dependent).

Methodology: A prospective cross-sectional study was piloted at Pakistan Atomic Energy Commission Hospital in cooperation with Pakistan Institute of Medical Sciences, Islamabad Pakistan. The study was done over a period of 6 months from April 2024 to September 2024. A total of 220 patients were included in the analysis, 110 of those were on dialysis and 110 were taking medicines .Different hematological and chemical parameters were assessed using Mindray BC-6200 hematology analyzer and Roche 6000. All these clinical and laboratory parameters were compared and analyzed.

Results: Mean creatinine level in dialysis dependent group (8.19  ± 2.68  mg/dl) was much higher as compared to drug dependent group (2.73 ±0.81 mg/dl). Hemoglobin levels were less in dialysis dependent group i.e 9.8± 2.03  g/dl as compared to those on medicines (11.7± 2.31). RDW levels were higher in the dialysis group as well and these high levels were associated with CVD and DM in the same group. MPV was higher in patients on drugs as compared to those on dialysis.

Conclusion: We found that high RDW is related with more CVD and DM, severe anemia, and inflammatory status i.e high CRP in patients with advanced CKD. Low MPV, on the other hand, appears to be linked to worse kidney function.
